Insight Global is seeking a Registered Nurse (RN) Case Management for a nursing job in Middlesex County, Connecticut.

This position will be full time, Monday-Friday, 8a-5pm, open to 4x10s as well! There will be ON CALL rotating weekends! As the Hospice Case Manager, you are responsible for: - In the field all day every day - Goes to office 1X a week for team meeting other than that on the road - Oversee 15-20 patients/ caseload - Benchmark is 5 patients a day o Outside of the day they go into the office they will probably only see 2 - Entering homes, nursing homes, assisted livings, wherever the patient lives - Making sure care of plan is appropriate - Seeing all patients 1X a week at least
